1. Two passport size photos
2. Personal check to US Department of Homeland Security ($410)
3. Form G-1145, E-Notification of Application/Petition Acceptance
4. Form I-765
5. Copy of TPS approval letter
6. Copy of recent passport
7. Copy of previous passport
8. Copy of Visa
9. Copy of new I-94
10. Copy of old I-94
11. Copy of work-permit.

2. Personal check to US Department of Homeland Security ($410)
3. Form G-1145, E-Notification of Application/Petition Acceptance

5. Copy of TPS approval letter

8. Copy of Visa

11. Copy of work-permit. (front and back copy)
12. WHAT happened to the letter your EAD card was attached to it when you received last time.
IMPORTANT INFORMATION-SAVE THIS NOTICE
 it says remember to tear-off portion to speed your application for extension or replacement card and it has 
A#  and three bar codes, Receipt#BARCODE, Card# BARCODE

Never applied work permit through TPS.
This is my first time I am applying work permit through TPS.
